Form of Particulars

This document is required by the administrator General for Jamaica to initiate investigations into the assets owned by the deceased person. The form of Particulars is also utilized to identify the possible beneficiaries of the deceased estate.

The Details of Claims Form

Where a creditor has a claim against an estate he or she is required to fill out the claim form in the presence of a Justice of the Peace. The form must be submitted along with any proof substantiating the debt to the Administrator General’s Office.

Affidavit in Proof of Death

This is a sworn document lodged with other documents to secure a Grant of Letters of Administration for a deceased person’s estate. The affidavit is used mainly to support the claim that the person in whose estate the grant is sought is deceased.
